1. Fill out Test Requisition and Payment Options form completely, checking desired tests. Please include all relevant clinical information, referring diagnosis, etc. Patient signature is required on payment and billing forms.
   
2. Place the specimen, forms, and any additional information in shipping container. (Call Emory Genetics Laboratory if you need shipping containers sent to your office.)
   
3. Ship via courier service within the Atlanta area, or via Federal Express if located outside of Atlanta. Samples can be received between 8:00 AM - 6:30 PM, Monday - Friday and 10:00 AM - 2:00 PM on Saturdays:
   
   
   • Local specimens are picked up on an as-needed basis at the physician's office by calling Emory Genetics Laboratory. Regularly scheduled pick-ups may also be arranged through Client Services, if desired.
   • Specimens outside of the Atlanta area should be sent by Federal Express by calling 1-800-GO-FEDEX (800-463-3339).
   • *CVS samples can be recieved M-F only. If received Friday, sample must arrive by 2:00 PM EST. PUBS samples (fetal blood specimens) should arrive STAT for proper processing.

There is NO CHARGE for Courier Services within the Atlanta metropolitan area.

International samples MUST be accompanied by payment in full. Emory Genetics Laboratory accepts Visa/Mastercard. We will also accept a check or money order made out to Emory Genetics Laboratory in US dollars. Institutional billing will not be arranged unless testing is to be ordered on a routine basis. Please contact our billing office for further assistance.

Emory Genetics Laboratory will initiate testing of a specimen when the following is received:

• A completed Test Requisition Form with patient signature and one of the following:
• A check or money order made out to Emory Genetics Laboratory, for the full amount of the test
• Valid credit card information with a signed patient authorization to bill for services rendered
