Georgia Medicaid: Billing

Emory Genetics Laboratory will initiate testing of a specimen when the following is received:

• A completed Test Requisition Form with patient signature
• A legible photocopy (front and back) of the patient’s Medicaid card

Emory Genetics Laboratory accepts Georgia Medicaid with a valid GA Medicaid card or number. Emory Genetics Laboratory does NOT accept non-Georgia Medicaid. Samples submitted with a “Medicaid pending” designation will be considered “Patient Pay” and arrangements must be made for payment before the testing will be performed.
