Cancellation Policy

Cancellation or revision of submitted test orders must be requested within 24 hours of receipt of the sample, following the guidelines below:

1. Contact Emory Genetics Laboratory Customer Service for cancellation or revision
2. A Test Cancellation Authorization Form will be faxed to the ordering institution for signature
3. Return the signed, completed Test Cancellation Authorization Form by fax to (404)778-8559
4. A set-up fee may be applied for processing of the sample prior to the cancellation request

Cancellation or revision requests received more than 24 hours after receipt of the sample cannot be honored, and tests will be performed, charged, and reports issued. This includes tests cancelled for billing (financial responsibility) issues. Tests expected to provide results within 24 hours (ex: STAT tests, prenatal Rapid Screen FISH, etc.) CANNOT be cancelled.
